Procedures for services

Scope of service for new patients

  • Providing health care services for child and adolescent psychiatric patients, both gender, aged not older than 18 years.
  • Service available from Monday through Friday
  • Services start at 7- 10 A.M. for new patients with no medical record or old patients who loss follow up more than 1 year.

New patient assessment (by appointment)

         Monday -Thursday      Patients aged range from 3 to 17 year 11 month olds

         Friday                        Patients aged range from 1 to 2 year 11 month olds

Service procedures for new patients

  1. Contact screening counter for application
  2. Receive hospital form and fill in the form
  3. Return the form at screening counter and wait for your child’s name to be called by a nurse for history screening. After screening, receive a doctor appointment card for next time of checking up.

** If screening result indicates urgent/emergent case, you will meet the doctor as the same day.

** If there is no patient at any appointment cue, you may meet the doctors as the same day.

  1. Referral case waits for your child’s name to be called at the payment verification counter, receive the void of payment document and show the document at cashier counter. You may have to pay some additional cost.
  2. If you meet the doctor, please follow the service procedures for patients with doctor appointment
  3. Paying by cash, wait for your child’s name to be called at cashier counter to pay for service costs.

Service procedures for patients with doctor appointment

1. Show an appointment card at screening counter, then guardian measures the height and weight of your child.

2. Wait for your child’s name to be called, then the guardian takes your child to be checked for vital signs and infectious screening

3. Wait for your child’s name to be called, the nurse asks you for case history of your child, then receive check card and doctor appointment card

4. Wait for your child’s name to be called, then show your payment privilege and wait for your payment privilege to be validated. Afterwards, wait at a doctor front desk

5. Wait for your child’s name to be called, show check card to officer and wait for validation, then receive doctor’s check-up cue card.

6. Wait for your child’s name to be called to meet the doctor (if you want a doctor certificate, service certificate, handicapped certificate and medical history, please inform your doctor beforehand)

7. After doctor’s check-up, wait for your child’s name to be called at appointment counter to receive appointment card and check card.

8. If you meet your doctor for the first time, please wait for the nurse for consultatio

9. Receiving medicine and service payment

                 -  If you have a medicine prescription, bring your check card to drug counter, wait for your child’s name to be called to pay your bill (if you have to) at cashier counter, and pick up your drugs.

                 -  If you don’t have a medicine prescription, bring your check card to cashier counter to pay your bill (if you have to)

If you want to postpone doctor appointment

  1. If you contact in person, inform your name-surname at medical record counter

Wait for your child’s name to be called to be asked for history

  1. If you contact by telephone to ask for a new appointment, call at number 0-2384-3381-3 Extend number 72117

          Guardian can call to change appointment using the same telephone number mentioned above

If you want to see the doctor before appointment schedule

  1. Inform your name-surname at medical record counter
  2. Wait for your child’s name to be called and then asked for medical history at consulting counter
  3. If your child in need to meet the doctor, follow service procedure for patient with doctor appointment
    1. Inform your name-surname at medical record counter
    2. Wait for your child’s name to be called at screening counter, fill in the application form and return with your identification copy and your signature on the copy to validate the document
    3. If you are contacted, visit the hospital and contact medical record
    4. Wait for your child’s name to be called to sign counter and pick up the certificate

Asking for medical history/ doctor certificate/ handicapped certificate

  1. Inform your name-surname at medical record counter
  2. Wait for your child’s name to be called at screening counter, fill in the application form and return with your identification copy and your signature on the copy to validate the document
  3. If you are contacted, visit the hospital and contact medical record
  4. Wait for your child’s name to be called to sign counter and pick up the certificate

Re-medication procedures

  1. Inform your name-surname at medical record counter
  2. Wait for your child’s name to be called at payment privilege validation counter
  3. Wait for your child’s name to be called and asked for history at re-medication counter
